Travel Route

At Summers Avenue Family Medical Practice, we make healthcare accessible to you by being centrally located between the Central Coast and Sydney, providing easy and convenient travel options for our patients.

On your arrival, our practice is located in a standalone building on the northbound side of the highway, in between a motor repairs shop and nursery. Enter via the driveway to access the main entrance.

Bus route

  • 595 – Hornsby to Mt Colah

  • 5-10 min bus ride  

  • Get off at 'Pacific Highway after Willarong Rd'  

Train route

  • Disembark from Mount Colah Train Station 

  • 10 min walk to Summers Family Practice

Car route

  • Short hop down the freeway from the Central Coast

  • 5 min drive from Hornsby

  • Easy access on Pacific Highway

Caring for our Patients

To protect vulnerable patients, masks are required for all patients ten years old and older who have respiratory symptoms (such as runny nose, sore throat, cough).

All patients with respiratory symptoms who cannot wear masks must wait outdoors in front of the practice or remain in their cars until their scheduled appointment.

Telehealth Eligibility

Unless you have an active Covid-19 infection, all Medicare eligible telehealth consultations require you to have attended our practice as a patient face-to-face within the last 12 months.

 

If you have not attended face-to-face within the past 12 months, a telehealth consultation can still be booked. However, standard charges apply with no Medicare rebate available.

To improve continuity of care, we encourage you to choose one of our doctors as your preferred doctor for long-term issues. There are many benefits to having a doctor who thoroughly knows your medical conditions. Our friendly staff will always aim to book you with your preferred doctor.

Our doctors may accommodate home and nursing home visits for their long-term regular patients if necessary.

 

Should your preferred doctor not be available, another doctor at our practice will be able to address your issue. We focus on working collaboratively at Summer's Family Practice to achieve the best health outcome.

Booking Your Consultation

We strongly recommend face to face attendance as the preferred means of facilitating a better quality consultation with higher accuracy of assessment.

Please book telehealth appointments if you have a confirmed Covid-19 infection or are symptomatic after having close contact with a Covid-19 infected individual.

If you have respiratory symptoms, please ensure a negative rapid antigen test within 24 hours of attending the surgery and advise our reception of your negative test result.

Cancellations

No fee is charged for appointment cancellations. However, please provide adequate notice of at least 2-3 hours where possible if your appointment is no longer required. This allows us to offer the appointment to other patients who may be unwell.

Appointment Length

Urgent & After Hours Care

While booking an appointment is usually essential, we will strive to accommodate urgent walk-ins, or if there are available appointment slots. For emergencies such as chest pain or suspected stroke, please immediately attend your local emergency department.

If you need medical attention outside of our regular business hours, including public holidays, please call the Home Doctor Service on 13 7425 (13SICK)The Home Doctor Service should provide our practice with details of any after-hours treatment to ensure our patient records are up to date.
