Quality care for the community
Dr David Lin joins Summers Avenue Family Medical Practice from 19 April 2021 at both Hornsby and Mt Colah.
COVID-19 Vaccinations: we will start administering the AstraZeneca COVID-19 Vaccine from the first week of April 2021.
Our reception staff will contact eligible patients to book vaccination appointments; please do not phone for a vaccination appointment unless we are unaware of your eligibility e.g. you are a health professional and have not advised us.
Please arrive 15 minutes before your appointment and print, complete and bring an Australian Government COVID Vaccination consent form if possible.
You will be advised about potential side effects and have an opportunity to ask questions. You will be asked to wait at the surgery for 15 minutes after your appointment to ensure there are no immediate side effects; this is standard procedure with ALL our immunisations.
For more information, visit the NSW Health website.
2021 Private and Government Flu Vaccinations have arrived at both Hornsby and Mt Colah surgeries. Please note, you need a two week gap between COVID and any other vaccinations.
Eligible patients for the Government Flu vaccinations include people over 65, Aboriginal and Torres Strait Islander people, pregnant women, patients with certain chronic illnesses (e.g. severe asthma) and children from 6 months-5 years old.
For more information, view our Services page.
At all visits, please:
Wear a mask,
Scan in using our Service NSW QR code (there is a manual sign-in sheet for patients without smart phones),
Advise reception staff if you have a fever, cough, cold, runny nose, sore throat or other possible COVID-19 symptoms (you will be asked to wait in your car or outside), and
Use hand sanitiser on arrival.
